A packet will require your whole team to work together. The housing application process has multiple steps. 

1. Planning and preparing- Start planning what support you/member will need to live alone
2. Update the Life Plan to reflect the decision to live independently.
3. Secure income funding (gather wages, apply for SSI, SSDI, etc).
4. Apply for community funding resources (Section 8, HEAP, SNAP)
5. Preliminary Housing Request (Budget, Life Plan, Attachment C(transition stipend), SNAP, HEAP, DDP2, CAS completed).
6. Save for the move, consider an ABLE account.
7. Contuine to develop supports for the move (staffing, medication/medical oversight, funding for move, etc)
8. Searching for the place (get a lease, renter's insurance, utility estimate).
9. Move into a new place with planned supports and staffing 
10. Final Housing Request (Apartment walk-through, Housing Agreement, lease, utility, renter's insurance, budget, Life Plan, SNAP, HEAP, SSI, Section 8, Staff action plans, broker agreement, and transition receipts 90 days before and/or after the move).
